2022 Appointment Dates

Scheduled appointments are held 12:00 noon - 3:00 pm, on select dates in the Human Resources Office in the Madison Municipal Building. Appointments may also be scheduled at your worksite, or another location convenient to you, by contacting the plan representative directly.
